Forms, templates, and checklists for arborists

Free tree risk assessment forms, arborist report templates, inspection checklists, and pesticide application record sheets, as printable PDFs.

Forms arborists actually fill in, as PDFs

Five downloads, each built from the form inside the PHC Rx app and exported to a printable PDF: an arborist report template with all twelve sections, a tree inspection report, a tree risk assessment form with both matrices, the assessment checklist for the truck, and a pesticide application record with every field the states ask for. Each page walks through what goes in every box and why, then shows the in-app version that fills itself in.

Use the paper versions to learn the structure, to train a new tech, or to keep a hand record on a job with no signal. When the third report of the day is still being typed up at 8 pm, the arborist report software page shows what the same twelve sections look like generated from the field record.

FAQ

Questions arborists ask

Are the templates free?

Yes. Download the PDF from each page, no email required. They are the same forms the app produces, exported blank.

Can I edit them?

The PDFs print and fill by hand. For a version with your company name, logo, and license numbers already on it, the app generates the same report from the field record.

What should an arborist report include?

Assignment, your credentials, tree identification, observations, findings, recommendations, products and rates, limitations, and the signature block. The template page explains each.

Which pesticide record fields are required?

Product, EPA number, rate, total applied, site, date, and applicator for every application, plus wind, temperature, and target pest in most states. The record template has them all.
